Clover Medical has developed a continuum of care wound care program that can greatly benefit your patients.
If a patient no longer qualifies for home health, then before discharging patient, we will make sure your patients receive the dressings they need.
We will ensure the dressings are reimbursed by your patients Insurance before providing.

Requirements before providing wound dressings
- Patient must be discharged from Home Health
- Patient cannot be on a Part A stay in a skilled nursing facility (patient can be on a non-skilled, not Part A stay in a nursing home or ALF)
- Patient must have Medicare Part B
We can have wound dressings shipped to your patient’s home within 24 hours.
We also have RN’s &PT’s that are Certified Wound Specialists available to answer questions.
